The LeadCenter Form Builder, page title Forms, lets you build inline or modal pop-up forms and embed them on your site. Submissions create contacts in LeadCenter with attribution.

Form Types

  • Inline — embedded directly in the page.
  • Modal — opens over the page when triggered.

Create a New Form

  1. Click Add new form at the top right of the Forms page.
  2. In the Create a new form modal, choose a template or start blank.
  3. The form builder opens. Add fields, organize them, and configure Lead Tracking Settings, including default Source / Campaign.
  4. Save.

Row Actions on the Forms Page

Each form's vertical-dots menu has:

  • Preview — opens the rendered form in a new tab.
  • Edit — opens the form builder.
  • Delete — removes the form.
  • Fields mapping — opens the custom-fields-to-LeadCenter mapping screen.
  • If the form is Modal:
    • Page load integration script — the script that opens the modal automatically when a page loads.
    • Button/link click integration script — the script that opens the modal when a specific button or link is clicked.
  • If the form is Inline:
    • Integration script — the script to paste where the form should render.

Best Practices

  • Always set a default Source on each form via Lead Tracking Settings.
  • Use UTM parameters in URLs leading to the form for paid-channel attribution.
  • Show a success message when you want to keep the visitor on the page; redirect when you want to drive a next step.
  • Use Fields mapping to map custom form fields to your LeadCenter contact custom fields.
  • Always test in incognito after publishing or changing scripts.
