Embedding LeadCenter Form Builder Forms

Once a form is created in the LeadCenter Form Builder, you embed it on your site using the integration scripts on the form's row in the Forms page.


Embed an Inline Form

  1. Go to Forms.
  2. Find your inline form in the table.
  3. Open the row actions menu.
  4. Click Integration script.
  5. Copy the snippet from the integration help modal.
  6. Paste it on the page where the form should appear.

Embed a Modal Pop-Up Form

Modal forms use two snippets:

  1. From the row actions, click Page load integration script. Paste this snippet site-wide on every page that should be able to open the modal.
  2. From the row actions, click Button/link click integration script. Paste this snippet on the button or link that should open the modal.

Verify the Form Works

  1. Open the page in incognito.
  2. Submit a test entry.
  3. Check the Form Submissions page and the Sales Pipeline for the new contact.

Best Practices

  • Install the page load script using a header/footer plugin so it survives theme switches.
  • Always clear caches after installing or changing scripts.
  • Test in incognito to bypass browser cache.
  • Confirm the form's default Source is set so submissions are not Unknown.
