The Financial Questionnaire (FQ) is a secure, step-by-step form you can use to collect detailed financial information from prospects and clients. Once a contact submits the questionnaire, you can review it and verify it. After verification, LeadCenter automatically updates the contact record with the submitted details — including spouse information and financial data like income, assets, and liabilities — so you don’t have to manually re-enter data.

Where to Find It

From the left navigation

  1. In the left sidebar, click FQ to open the Financial Questionnaires list.

Note: The FQ menu item is available on Financial Advisor accounts.

From a contact record

  1. Open a contact.
  2. Go to FinancialsFinancial Questionnaire.

How to Use

Workflow:

SEND LINK → CLIENT SUBMITS → REVIEW & VERIFY

Send the questionnaire to a contact

There is no “Send” button. Each contact has a unique FQ link that you share with them.

  • Option A: Copy the link from the contact
    1. Open the contact record.
    2. Go to FinancialsFinancial Questionnaire.
    3. In the header, find the public FQ URL and click the copy icon.
    4. Paste the link into an email or text message.
  • Option B: Copy the link from the FQ list
    1. Go to FQ from the left navigation.
    2. Find the contact in the list.
    3. Use the URL column to open or copy the link.
  • Option C: Use the email shortcode (recommended)

    When composing an email to a contact inside LeadCenter, include the shortcode [[fq_url]]. LeadCenter will automatically insert the contact’s unique FQ link when the email is sent.

Important: Each contact has a unique questionnaire link. Do not send the same FQ link to multiple contacts.

What the contact experiences

  • Email verification — the client requests a one-time code sent to their email and enters it to access the questionnaire
  • Save and resume — they can save progress and return later
  • Review & submit — on the final step, they review their entries and click Submit
  • Session timing — once verified, the client session remains active for up to 12 hours

Where to Find Results

On the FQ Listing Page

The Financial Questionnaires list helps you manage all submissions. You can search and filter by status, contact owner, month, and year.

If you want to return to the full list, click Reset Filters to clear all active filters at once.

  1. Go to FQ from the left navigation.
  2. Apply any filters you need to narrow the questionnaire list.
  3. Click Reset Filters to remove the active filters and show the full list again.

Tip: Use Reset Filters if you cannot find a questionnaire you expected to see. A filter may still be limiting the list.

On the contact record

Open the contact and go to FinancialsFinancial Questionnaire to review status and take action. You can also see helpful timestamps such as when the client opened the questionnaire and when they last updated it.


Verification

Verification is the step that writes questionnaire entries into the contact record. Only questionnaires in Submitted status can be verified.

  1. Open the contact and go to FinancialsFinancial Questionnaire.
  2. Click Verify.
  3. Add your Notes (required).
  4. Click Verify Submission.

Tip: Use Preview & Print to review the full submission before verifying.

After verification, LeadCenter will:

  • Update the contact record with verified entries, including spouse details and key financial information (income, assets, liabilities, and more)
  • Create or update a spouse contact record (when applicable) and link the relationship
  • Record Verified by and Verified at for audit history

Download a Blank Printable PDF

You can download a blank printable version of the Financial Questionnaire directly from a contact’s Financial Questionnaire tab. This is useful when a client prefers to print the form, complete it by hand, and return it to you offline.

  1. Open the contact’s detail page.
  2. Go to FinancialsFinancial Questionnaire.
  3. Wait for the page to load.
  4. Click the green Download as PDF button in the top-right area of the card.
  5. The blank questionnaire opens in a new browser tab, where you can review it and download it.

Note: The Download as PDF button shows a blank printable questionnaire. It does not depend on whether the contact has already submitted an FQ.

What the Blank PDF Includes

The blank form includes all Financial Questionnaire sections formatted for paper completion, including:

  • Concerns
  • Objectives
  • Risk / Goals
  • Declaration & Signature

Notes About the Blank PDF

  • The downloaded file is always a blank form. It does not include the contact’s submitted answers.
  • The PDF file name is Financial-Questionnaire.pdf.
  • The PDF opens in a new tab. Use your browser’s normal download button to save it.
  • This button is for internal staff use. Clients do not have access to it.

Document Uploads

Contacts can upload supporting documents through the questionnaire portal. Uploaded documents are linked to the contact’s record in your document storage area for easy access.

Host the Secure Client Portal on Your Domain

LeadCenter can host the Secure Client Portal on a branded URL using your own domain (for example, fq.yourdomain.com). This gives clients a trusted, familiar link for accessing:

  • The Financial Questionnaire
  • Secure document uploads

Requirements

  • An active subscription (or offline / LeadCenter plan) is required to generate SSL certificates.
  • SSL certificates are generated only for custom domains, not default LeadCenter domains.

How to Set Up Your Financial Questionnaire Domain

Open the setup page: Settings → AUM & insurance settings tab → Financial Questionnaire Settings → Configure financial questionnaire domain. You can also access it directly at app.leadcenter.ai/dashboard/financial-advisor/fq-domain. The page is titled Configure Financial Questionnaire Domain and guides you through three on-screen steps.

Step 1 — Enter your domain and save

Enter your subdomain, such as fq.yourdomain.com, in the domain field and click Save Domain. LeadCenter adds the https:// prefix automatically.

Step 2 — Create the domain CNAME at your DNS provider

At your DNS provider, add a CNAME record pointing your subdomain to us-domain.leadcenter.ai:

If your DNS provider supports proxying, such as Cloudflare, set this record to DNS only.

Step 3 — Add the SSL validation CNAME

After saving your domain in Step 1, wait about 30 seconds and then refresh the page. The SSL validation CNAME appears under Step 3 in a table that includes Type, Name, Value, Proxy, and Status.

Copy the Name and Value from that table into a new CNAME record at your DNS provider. If your provider supports proxy settings, use DNS only.

The Status column updates automatically as AWS validates the record. Verification usually completes within a few minutes, but it can take up to 24 hours. You have up to 72 hours to add the validation record before the certificate request is marked Timed Out.

Certificate Status

  • Pending — waiting for DNS validation
  • Issued — the certificate is active and your FQ domain is secured with HTTPS
  • Timed Out — the validation record was not added within 72 hours
  • Failed — the certificate request failed
  • Expired — the certificate expired

Reissuing a Certificate

If the status is Timed Out, Failed, or Expired, a Reissue Certificate button appears on the page. Click it to request a new certificate. If the DNS validation record is already in place, the new certificate can validate automatically without additional DNS changes.

Note: Reissuing a certificate also requires an active subscription.

Important Notes About Custom Domains

  • The SSL validation CNAME must be added within 72 hours of certificate creation.
  • If you change the domain later, LeadCenter automatically requests a new certificate for the new domain.
  • If you switch back to a previous domain, LeadCenter can reuse the earlier certificate when available.

Troubleshooting

  • The client didn’t receive the verification code: Confirm the contact’s email address is correct and ask the client to check spam or junk folders. They can request a new code if needed.
  • I don’t see the Verify button: The questionnaire must be in Submitted status. Draft questionnaires cannot be verified.
  • I need to correct answers before verifying: Use Edit to update the submission, as long as it has not been verified yet, then verify.
  • A verified submission was deleted: Deleting a submission does not undo the data that was written to the contact record during verification.
  • My custom domain certificate is not issuing: Make sure you added the SSL validation CNAME exactly as shown on the page, refreshed after saving the domain, used DNS only if your provider supports proxying, and confirmed your subscription is active.
  • The blank PDF button is missing: Open the contact’s Financial Questionnaire tab and wait for the page to finish loading.

FAQs

Can I edit a questionnaire after it has been verified?

No. Once verified, the questionnaire cannot be edited. Use Preview & Print to review before verifying.

Can a client submit without completing every step?

Yes. A client can submit without completing every step. You can review step completion status on the contact’s Financial Questionnaire tab.

Can I fill out the questionnaire on behalf of a contact?

Yes. If available on the contact’s FQ tab, use Edit Submission to pre-fill or enter information on behalf of the contact.

Is the downloaded PDF pre-filled with the contact’s answers?

No. The downloaded PDF is a blank printable version of the questionnaire.

Do I need to contact support to set up SSL for my FQ domain?

No. SSL certificate creation and validation are handled automatically. You only need to add the DNS records shown on the page.

Do I need to refresh the page to see the SSL validation record?

Yes. After saving your domain, wait about 30 seconds and refresh the page so the SSL validation CNAME can appear under Step 3.
